Ulster County Transition Council:
preparing yourself for life after high school
The Ulster Country Transition Council is a group of voluntary members representing Ulster County NY schools, post-secondary education programs and adult service-providing agencies. Our goal is to inform school districts, students and families of available post-school options and the means to obtain services. We meet monthly to share strategies that improve the transition planning process for students with disabilities.
We strive to assist students to reach their full potential and successfully live, learn and earn in their communities.
Mission Statement: Representing the greater Ulster County NY area we are committed to providing quality transition services for youth with disabilities leading to successful adult outcomes. We agree to identify those services and communicate, advocate and coordinate each to the vested community.
