Who's on Your Team? Find out who will be serving on your transition team and get a list of contact information and a meeting schedule. Remember, you (and your family) are the central members of the team. A Transition Team can include you, a special education teacher, a regular ed teacher, a social worker, school counselor (guidance), speech & language pathologist, psychologist, outside mentor, occupational & physical therapist, administrator, CSE Chair(Comittee on Special Ed) and others.
